They may have hit the top with a small hammer, to make a mark on sheet metal, prior to drilling a hole through it. I can not see the top, so as to ascertain whether it has been hit.

Probably a scribe . Some combination squares come with a small scribe in them.

I'm thinking it's a hand awl for punching holes in leather.

Now, as to WHY that comes to mind.... I have no idea!!
