need help/confirmation, victorian sash buckle?

Hey a friend has this and was wondering if i knew what it was. i thought maybe a sash buckle but am not sure. What do you guys think?

apparently the loop is flat, he does work on watches so if he says its flat I believe him. I am thinking either scarf or sash buckle type thing

Interesting piece that there appears to be a hole(?) on the reverse side that you can see the head through. That makes me wonder if 2 different pieces of jewelry were combined into one. Plus I wonder if the ring is original. If so, I'd think it would have to be worn out of sight.
