A "time and place" thing ? Would a wife's initials be used on silverware ?

A (very) long stretch ... Paul Goodloe McIntire, a wealthy philanthropist, married Edith in 1891, and went on to join the New York Stock Exchange in 1901. A time period when monogrammed Elltonhead silverware would have been a thing for the wealthy.

E. McIntire ?

Just tidying up some blasts from the past (including some very ancient ones), largely for the benefit of anyone searching the site for information.

The spoon itself is rather older than that. The maker Thomas Eltonhead of Baltimore, Maryland was active between 1835-1843.
