As far as I know there has never been a firearm that could chamber a 12 gauge shell which used a rimfire ignition.
Maybe someone buggered it up trying to remove the primer to reload it without proper tools?
Some old break open shotguns had firing pins that would stick down, or pin is too long. then when you opened it, it would drag across the shell causing what you see here. I have seen this often as I do Gunsmithing
