This item is Too Small to be a suspender clip. What size would the button be on the front to hook it to?

The item is a sock or garter clip. 100% certain.

Buckle boy.....The image that you marked is from what are known as a hook and eye, like the ones that are (say for example) on a garment. The one shown is a garter or suspender clip, and yes the buttons are very small for them.

It would have to be a very small button. I would also think sock or garment garter.

Are you sure that is a suspender clip? Based on its size and the rivet arrangement, is it possible that it is a catch off of a leather document case or perhaps a book strap?

I know this is an OLD OLD post, but, since this post was made, I have seen these things pop up and are always misidentified as suspender clips or sock clips. What I have discovered is that they are "busk hooks" off of a corset.

I'm about 99% sure the one the red arrow is pointing at is actually a cape hook, or cape clasp, and is supposed to be bent like that.
The one in the OP is indeed a garter belt clasp.

The traces of rust on the item suggest that it was fastened to the steel rib of a corset. A suspender clip or garter clip would not have this rust.

Not to be argumentative, but those garter clips shown in your photo are completely different than the one the OP posted.

I just used the photo as an example, I wasn't trying to match it to that particular garment and manufacture. Obviously it's much older.
Sometimes folks are a bit too focused on finding an exact match with patent and serial numbers, but unfortunately sometimes all you can get is 'close enough'.

When you said, "The one in the OP is indeed a garter belt clasp" you seemed to be pretty definitive in that as an ID. Can you find a garter belt clasp that matches? The busk hook pretty well matches, but I can't find a garter belt clasp that does.
