Possible quarter mint malfunction?

Found this quarter when counting out my sons change for him. It is a 2001 North Carolina. Has anyone seen anything similar?

Also appears to me as it was being polished when an error occurred. Unlike the looks it feels very smooth.

I agree with the post mint damage, coins are not polished at the mint.
