Good call on the half real (16.4 mm) versus the dime at (17.91mm) .
A 178? (Charles) coin could be either C III (to 1788) or C IV (from 1788).
Mexico City mint.

Edit: Looking at the coin again, I read most of the name: CAROLUS. This only appeared on the 1/2 R of Charles IV and his coinage of the 1/2 real began in 1789. CAROLUS III 1/2 reales had only CAR and were not minted beyond 1771. So if you can read 178X, then X is a 9 and it's from C IV.

Its a refilled hole/holes.

Nice Find! It looks like (on my monitor) that it may be a "4".
