I haven't been able to find it exactly but it is possibly circa 1500s Italy. The reverse looks a lot like the arms of the De Medici family. Here's a similar design-

Yea, That looks like it! the guy is facing the opposite direction but that is really close. Found this one the old fashion way, bought a wallet at a garage sale and it was tucked inside.

I think the legends match the Cosimo I Crazia. It might be a different year? Is your coin about 1.1g and 22mm?
