Shape reminds me of some Schuylkill County PA Dog Licenses ,

However unless you photocopied the Back instead of the front,
I don't see anything on it, to ID?

Aha! I knew I had seen that shape before and lo and behold,we have two in a Riker case my husband found years ago.Nice find stillrollin,hope you don't mind me posting my pics here.
