Is it slotted like that on both sides? If so maybe a size/number 10 zipper pull or, fastner/cinch where two strings are slipped through to tighten.

It's part of a Weight Watchers key chain, the number 10 meaning one had lost 10% of their starting weight. See following link, second picture down. We can mark this one solved I guess!
