No info on maker but I think this might be the same pattern fork in a eBay listing in google images. I can't get the ebay link to show up but I did find this reference which I think is the same item -

Vintage Sterling Salad or Vegetable Fork Crown on end of Handle ,Centre Hall, Pennsylvania, United States ,QMstore.com

“In good vintage condition with normal wear consistent with age. Bowl does have a light gold wash. Is monogrammed with the letter D.”
Composition:Sterling SilverType:Serving Fork
Offered is a lovely sterling silver serving fork, which is either a salad fork or vegetable fork. It is marked sterling on the reverse side. Measures 7 inches long. Bowl measures 2 1/8 inches across. Gram weight is 32.
