The hand-written parts are tough to read, but his place of residence (Wohnort) seems to be given as “Weidenau”, which is a suburb of the city of Siegen. There’s a street (Strasse) given, but I can’t read it.

At the bottom, the print reads “In the case of official questions, the number of the license and badge must be stated.”

In his photograph, he appears to me to be wearing a police uniform.
