I've run into this type of trim around the vent pipe from stoves or heaters. I could be wrong but it could be too small of an opening for a vent pipe.

At 4" it seems to be too small for a wood stove pipe escutcheon....

Gas heating stoves used smaller vent pipes than most wood or coal stoves. 4 inch was a common size pipe .
