The 40 and 8 organization referred to these boxcars that carried 40 people or 8 horses. (note writing on car)
This was one of their medals, if I got it right.

It seems that the organisation had its roots in 1920 for WWI veterans as an honor society for certain members of the American Legion. By 1929 it had become rather more than that, contributing to all kinds of social benefit and charitable activities, which it still does today. Currently it has more than 20,000 members and membership is by invitation only to Honorably Discharged Veterans and all Military Personnel currently serving the United States Armed Forces, whether they have seen combat service or not.

The only significance of the date March 11, 1923 is that it’s when this medal design was patented... it's not a foundation date for the organisation, nor an issue date for the medal.
