Looks like it was a square wrench...maybe to a gas or steam valve. :dontknow:

Looks to me like a ratchet type wrench .. When worked in the opposite direction it slips, so it has a ratchet effect...

Looks like the handle to a plug valve, or some other type of valve. Used in everything from oil and gas to water and sewage pipes.
This one is similar, although not square on the end. I have seen square ones though.
