I am not sure what this is? It looks like a guy holding a baby with a cane? Maybe its Jesus.

It's hard to say. Essentially the same design has been in use for centuries. However, these medals are often struck or cast in base metals and alloys which are highly susceptible to corrosion: pewter, white metal, zinc, etc. So, the condition of this one is not necessarily indicative of great age.
