have no idea what this is. Disk is concave and slightly larger than a quarter and does not slide off the nail.

You are correct. It is just a washer held in place by the nail.
Some time ago someone posted info in a thread about people using half cents as roofing washers as they were cheapers than buying washers. Not sure it's true, but that's what they said. It was thread about half cents.
