watch fob. but...that chain looks to be a recycled sword hanger, i have no idea on the medallion.

A friend of mine from out of town is going to find out for you. She studies old antiques, etc. I will let you know what she thinks

thanks all help would be great. I've had this a long time never was sure what it is also if it helps the other end has a belt loop on it.

i changed my mind. if that medallion is a belt hook, it is a sword hanger.

I believe it's a hanger from a sword belt worn by a member of a fraternal order. It's not the same order, but this sword belt from the Uniform Rank of the Knights of Pythias will give you an idea of what I mean...
