It looks like a heel tap. As far as the size is concerned, back during Victorian times, people were much smaller and shorter, thus had smaller feet. The average waist size for a Victorian woman was 18 inches. The average waist for a woman today is 34 inches. This tap could have been for a woman or child's shoe, but since I don't know the dimensions, I can't say whether it would have fit man, woman, or child. Below is a pic of a stirrup Garabaldi found back in 2009. Note how small the stirrup is in relation to Garabaldi's shoe.
