I may have one: How about not some royal family but a triandry of politicians or philosophers or something like that?
Couldn't find an accurate match but the classic socialist triad Marx-Engels-Lenin comes pretty close...

I think you are right on. It is a Soviet era medallion.

Hard to say the age. The pic I posted was said to be from the 1960's. Here is one from East Germany in the 1970's.

I saw stuff from the 30's - 50's with these three guys AND Stalin included. Stalin seems to have dropped off after his death. All the items are socialist/communist idealogy related. There are plenty of organizations that might have used a medallion like yours on a plaque or a medal. Without any other clues, all we can do is speculate.
