BREAKING: Helicopter crash in the Hudson River | LiveNOW from FOX

Initial reports suggest it was a “recreational” helicopter, that may have taken off from the Manhattan area. Two adults and three children said to have been on board, with two people later pulled out of the river in an unknown condition and sadly one confirmed fatality (later reports say three confirmed fatalities).

One eye witness reported seeing one of the chopper’s rotor blades detaching immediately before the crash. Later reports say the main rotors came off and and the tail with tail rotors was severed.

Chopper seems to be registered as N216MH, a Bell 206 L-4 LongRanger owned by Meridian.

It's tragic. Latest reports say an entire family of Spanish tourists (two adults, three children) and the pilot have all been killed.

The chopper was owned by Meridian, a small company specializing in refurbishing, brokering and leasing helicopters. It was being operated by New York Helicopter for sightseeing tours.

Sadly, New York Mayor Eric Adams has confirmed the death of three adults and three children (a visiting family of tourists from Spain and the pilot). Four were pronounced dead at the scene, and two others were taken to area hospitals where they succumbed to their injuries.

It appears that a rotor blade had come loose, with the instability causing the two-blade rotor to completely detach from the aircraft together with its transmission and slice though the tail. The main part of the chopper landed upside down in the water, with the rotor assembly following it down.

It may or may not be significant that the NBC4 channel reported that its own helicopter, hoping to get news footage from the air, was unable to take off because of conditions, with the weather in New York described as windy, under thick cloud cover.
 

The spinny things don't come loose very often, so I am guessing some maintenance engineer is going to be having a sleepless night.

don't know who but Two passengers were reportedly alive when divers pulled them from the Hudson but later died, New York City police commissioner Jessica S. Tisch said at a news conference.
 

Helicopters DON'T just fall from the sky - What is "Mast Bumping"?​


 

Mast bumping is indeed a possibility but most usually the rotor blades detach relatively cleanly, not with the transmission still attached to them as seen in the footage. The transmission usually (but not always) stays within the hull.

Another strong possibility is that the main rotor blades struck the tail boom and sliced though the hull causing the transmission to separate with the blades still attached to it. Tail boom strikes are a known 'issue' in helicopters of this type that can arise from abrupt control inputs (something pilots are trained to avoid, but may still execute in a knee-jerk response to avoid something such as a bird-strike, unexpected low cloud cover or proximity to terrain) and also from a catastrophic transmission failure.

Slicing through the tail seems to be apparent from the footage.

At this early stage in the investigation, this rather long video tells us very little but, at about 21:43, the NTSB chair says that she is aware of reports of a large flock of birds in the area at the time of the crash.

It wouldn’t necessarily take an actual bird strike to cause this kind of accident. As I mentioned in post #18, a very abrupt control input to avoid a flock of birds could cause a tail boom strike from the chopper’s own main rotor blades.
