Off the cuff I'd say its not part of a medal, only because the fastner that holds the parts together would more likely have been just a metal loop, like in the photo below. It seems to me this was constructed stronger because it needed to be for whatever its use (maybe for a strap?). I'll research further, though.

Is there another name on it? I can see where its marked "& Smith"...can you make out what the first name is?

It's a clothing fastener. I have a number of different kinds of clips, straps, hooks, etc. that are related to clothing.
Your piece can be found with different designs and/or maker marks and can look nice when displayed with other types. Quite often I unearth similar looking clips(with teeth) that say PARIS 1870. If I get motivated, I could take a pic of all the varieties that I have of your fastener.
Your find adds to the fun in learning about the past!

Not to say it couldn't be something else. But if it's about 2 inches wide. Like Cheese said, I think it might be a suspenders cinch (adjuster). I've got a couple I've dug that are similiar to that one. One has little teeth for grip and the other one is smooth, like yours appear to be. It's hard to tell, but I think the year on it is a patent date. Generally they'll have a Pat. before the date.

I agree with cheese and RV, suspender holder was going to be my guess, my grandfater wore them all the time. HH
