People like to do things with cents. That appears to have been set into a bezel - perhaps it originally had a ring on the rim to hang on a charm bracelet? Anything significant happen in 1942 with your grandparents?

I've seen a die capped penny but that almost looks like a penny stuck in the die. Is it thicker than a normal penny?

Yes. According to him its 3 or 4 times thicker than a penny.
 

Must be two wheaties in the wide bezel then.

A crafted jewelry piece, pretty neat. Is not 1 solid item, separation can clearly be seen, especially in the obverse.

I have not personally handled it but there must be multiple coins. I do see a crack around the edge in the pics. Is it possible this is some kind of striking error stacking multiple couns?

12.1 grams would make it around 5 times the weight of a penny?
 

- 0 chance this is a "mint error", it is what it is, a decorative piece fashioned by someone well after it left the mint. For starters, there is no process at the mint that could or would produce a monster like this, think about it logically.
People post created oddities like this all the time and most refuse to believe its anything else but a valuable and rare error, guess the only way top get a definitive answer is send it into PGS - But going that route costs a nice chunk of change, only to be told the same thing being said here.

Seattle used to have pennies imbedded in a decorative concrete wall along the waterfront. From time to time people would try to remove some of the older dates. Perhaps this was pried out of something like this.

Well, Charlie posted something very much like it, though your example looks better crafted. I have seen quite a few over the years on forums and at coin shows, people were wonderfully creative back then and they often encased coins in buttons, cameos, cuff links ect. This style due to its weight may not have been optimal for wear, really doesn't necessarily have to have a purpose but obviously someone decided it was worth the time and effort to create. Wear on the high points of the obverse [and it would stand to reason that it was in mint condition when fashioned, don't think anyone would use a trashed, circulated coin for something like this] suggests it was pressed in, and a very nice fit.
