It looks like your guess of Sunday School was correct. It was probably an award for good attendance. Here's an excerpt from a story related by a member of the Methodist Church in Hayesville Ohio.

"My parents belonged to this church and my father was the secretary/treasurer to Sunday School so he was always there in the church. We had The Cross and Crown system for regular attendance. I got up to 9 years continuance attendance. We were allowed two Sundays a year which you could make up if someone came to visit you."

From the rest of his story, it looks like he was in Sunday School around the late 1920's.

Thanks very much for all of the valuable info - my guess is that it dates from the 1920s or so. There is a Reformed Church just down the road! Thanks again for the very quick ID!
