It's quite possibly both. It looks to be a cast counterfeit, but when casting someone decide to add a shank and make it into a button.

It started as a (fake?) Mexican coin; either as a 1/2R (12mm), 1R (20mm), 2R (28mm) or 4R (34 mm) depending on its diameter. Based on the image provided, it could have ended up as a button. Future pics with something beside the object to show relative size (or stating the size) would be helpful.

What he said, coin buttons are always blank on the underside, at least over here they are. So I would say yours as been turned into a button, from what was once intended to be a coin.

ok thanks but ! I took it to a coin shop, the guy has an awesome scope plugged into his computer . showed that definitely a coin first , not a fake and has the english shield and pillers on the reverse side , is some sort of copper/bronze, is 1805 period , BUT someone tried to make it a button, so IRON PATCH is right ! IM happy because its my oldest coin i have metal detected,,, YES !! So its a SPANISH COLONIAL CAROLUS 4 REALES COIN.
